Per the Centers for Disease Control, nearly half of adults in the United States have hypertension or are taking medication for hypertension, and only about 1 in 4 adults with hypertension have their condition under control. High blood pressure, also called hypertension, is blood pressure that is higher than normal. Your blood pressure changes throughout the day based on your activities. Having blood pressure measures consistently above normal may result in a diagnosis of high blood pressure (or hypertension).
High blood pressure usually develops over time. It can happen because of unhealthy lifestyle choices, such as not getting enough regular physical activity. Certain health conditions, such as diabetes and having obesity, can also increase the risk for developing high blood pressure.
The higher your blood pressure levels, the more risk you have for other serious health problems, such as heart disease, heart attack, and stroke.
That’s why we’re doing everything we can to help our hypertensive patients get their blood pressure under control and live healthier lives. With that in mind, we’re offering a limited number of patients the opportunity to participate in a new Remote Patient Blood Pressure Monitoring Program.

The iBloodPressure Cellular Connected Blood Pressure Monitoring device is patient-friendly, simple and easy to use, transmitting blood pressure results to your Community Health Northwest Florida healthcare provider in REAL TIME…from the convenience and comfort of your own home!

- 6 month program
- One-on-one device training
- Personalized case management
- Quarterly meetings with your provider via telehealth or in person to review progress
