Sliding Fee Program
What is it?
Our sliding fee program is a healthcare assistance program that helps you afford the care you need. Your ability to pay should not stop you from receiving the health care you need. The program utilizes a sliding scale based on your income and family size. Those eligible for the sliding fee program may receive reduced cost healthcare services, depending on your individual situation.

Community Health Northwest Florida offers this sliding fee program as a Federally Qualified Health Center (FQHC). Our local, regional, state, and federal funding sources help to offset the costs of providing care to those most in need.
Who is Eligible?
Anyone can apply for the sliding fee program. Interested participants may talk with a Financial Counselor about their eligibility requirements. Examples of those eligible for application include:
- Uninsured or Underinsured Individuals
- Insured Individuals with a high deductible
- Insured Individuals with limited or uncovered services
