CHNWF Milton Moves to 6671 Caroline Street, Expands Services

Community Health Northwest Florida, the area’s only Federally Qualified Health Center, announces the opening of its newly renovated Milton patient care site at 6671 Caroline Street.

Beginning Monday, August 9th, patients who were previously cared for at Community Health’s longtime Milton location at 5520 Stewart Street will be seen in the newly renovated 25,000 square foot building on Caroline Street, which provides the space needed to expand health center services for residents of Milton and Santa Rosa County. The move offers additional space for existing Adult Primary Care, Pediatric Care and Behavioral Health and Wellness practices, and allows the health center to add Dental Care and a Pharmacy.

“This is the perfect opportunity for us to expand our capacity to better serve not only our existing patient base, but offer services to residents of the Milton area at large with the addition of much needed Dental Care and a convenient onsite Pharmacy,” said Community Health Northwest Florida Chief Executive Officer Chandra Smiley. “We’d like to thank all who have partnered with us to make this dream a reality, including Ed Carson of Carson Lovell, Inc., Mickelson Construction Services, Inc., and Scott Holland of R. Offering both in-clinic and telehealth virtual visit options for most services, Community Health Northwest Florida accepts Medicaid and Medicaid Managed Care plans, including Simply Health Plan, Humana, Sunshine Health and WellCare, as well as most commercial insurances. The health center also provides a sliding fee scale plan for uninsured patients.

Since 1992, Community Health Northwest Florida has served residents of Escambia and Santa Rosa counties and surrounding areas. Since 2007, it has been the area’s only Federally Qualified Health Center (FQHC) as designated by the Department of Health and Human Services, Health Resources and Services Administration (HRSA). During 2020, the health center cared for 50,779 patients representing 150,000 patient visits.
