Community Health Northwest Florida is a Federally Qualified Health Center and a Federal Tort Claims Act (FTCA) Deemed Facility. This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

Community Health Northwest Florida provides access to interpretation services for health center patients with limited English proficiency.

Our after hours on-call nurse is available for phone consultation after Walk-in Care hours (after 8 p.m. on weekdays and 6 p.m. on weekends). To contact us after hours, please call our main site number at 850-436-4630.   In case of a medical emergency, please call 911.
