We are proud that our own Sandra Donaldson, Director of Special Programs, authored an article published today by the CDC Center for Disease Control and Prevention. Her article appears in Preventing Chronic Disease (PCD), a peer-reviewed public health journal sponsored by the Centers for Disease Control and Prevention and authored by experts worldwide.

“Community-Centered Health Home: Life on the Other Side of the Wall” explores CHNWF’s work as a Community-Centered Health Home, and highlights an innovative program targeting hunger and substandard housing for residents of the Oakwood Terrace housing development. It’s a great read!
