Community Centered Health Home
Community Health Northwest Florida, a Federally Qualified Health Center (FQHC) based in Pensacola, Florida, was one of five Gulf Coast community health centers (CHC) to participate in the Louisiana Public Health Institute’s (LPHI) Community-Centered Health Home (CCHH) Demonstration Project. The CCHH model was developed by the Prevention Institute as a framework for CHCs to engage in creating safer, healthier, and more equitable communities.
The CCHH model helps to support the Weis Community Partnership School. Community Health Northwest Florida acts as one a core partnering agencies under the twenty-year agreement to embed healthcare, behavioral health services, and social service support directly inside the elementary school. Our role as a core partner has been to address health care needs of students by offering a pediatric primary care practice within the community school.
We partner and organize community and civic events throughout both Escambia and Santa Rosa counties. Our mobile medical unit team conducts health care screenings and Outreach and Enrollment Services to residents in targeted communities.
As a Community Centered Health Home (CCHH), we seek to reduce non-medical barriers by addressing health inequity. CCHH begins outside of the walls of healthcare by utilizing community prevention resources to improve health, safety and equity outcomes.
Our new main site will be located at the former Allie Yniestra Elementary School. This new home supports our mission and culture by rooting its services directly into a community to help identify and support social determinants and disparities of health care such as poverty and access to care.
