Community Health Northwest Florida serves on the Live Well Planning Team and Steering Committee.  Live Well Partnership for a Healthy Community, in conjunction with local partners,  has released the preliminary findings from its Community Health Needs Assessment.  The study included a survey of over 2,100 community residents, a survey of community leaders and analysis of the leading causes of death and poor health in our two-county area.  This needs assessment gives us invaluable information on the specific needs of the citizens in our area and how we can better serve the community!  Explore the findings of the most recent study here .
