13 locations receive Patient Centered Medical home-recognition

This week we received word from the National Committee for Quality Assurance that 10 Community Health locations sustained their Patient-Centered Medical Home recognition while three new locations were added.

NCQA recognizes patient-centered medical home as a model of care that puts patients at the forefront of care. PCMHs build better relationships between patients and their clinical care teams. Research shows that PCMHs improve quality and the patient experience, and increase staff satisfaction—while reducing health care costs.
