12th Avenue Pediatrics is back and better than ever

Community Health Northwest Florida’s 12th Avenue Pediatrics location reopened to patients on February 23, 2026 — 10 weeks after a pickup truck inadvertently crashed into the lobby of the East Hill facility.

No one was injured in the December 17, 2025, accident.

Patients’ appointments were moved to other nearby Community Health Northwest Florida locations following the crash. Those locations included Trinity Pediatrics at 5375 North Ninth Avenue, the pediatrics clinic at 2315 West Jackson Street, and Adult and Family Care at 1400 North Palafox Street.

“First, we continue to be grateful that nobody was hurt,” says Chandra Smiley, CEO of Community Health Northwest Florida. “The second miracle was how quickly our scheduling staff began contacting patients to make sure their care was not interrupted. And I can’t say enough about our facilities team, which began lining up inspections and ordering parts that same day.”

Once the building was deemed structurally sound, a new door and front windows were ordered, which remained boarded up during the re-construction process. The front brick facade also was repaired, new flooring was installed, and new paint hit the walls.

Patients also will see yellow bollards in the parking lot — another measure to help ensure the continued safety of everyone inside the building.

“We’re extremely excited to get everyone back into 12th Avenue,” Smiley says.
