Patients’ Rights

As a patient of Community Health Northwest Florida, you have the right to:

  1. Respectful care. Receive considerate and respectful care in a safe
    environment, free from all forms of abuse, neglect, or harassment.
  2. Privacy and confidentiality. Have your personal health information protected
    in accordance with applicable federal and state law, including HIPAA. Review our Notice of Privacy Practices.
  3. Information about your care. Be informed of your diagnosis, treatment options,
    and expected outcomes in language you can understand. Request an interpreter at no cost if English
    is not your primary language.
  4. Informed consent. Receive information about proposed treatments or procedures
    — including risks, benefits, and alternatives — before consenting to or refusing care.
  5. Participate in your care. Be involved in developing your treatment plan and in
    decisions about your care, including the right to refuse treatment and to be informed of the
    consequences of that refusal.
  6. Access to your medical records. Request and receive a copy of your medical
    records in accordance with applicable law.
  7. Know the cost of care. Receive information about fees for services and our
    Sliding Fee Discount Program, which reduces costs based on household income and family size.
  8. Nondiscrimination. Receive care without discrimination on the basis of race,
    color, national origin, age, disability, religion, sex, or any other protected characteristic. View

  9. File a grievance. Voice concerns or complaints about your care without fear of

  10. Advance directives. Formulate advance directives (such as a living will or
    health care surrogate designation) and have them honored within the limits of the law.

Patients’ Responsibilities

To help us provide the best possible care, we ask that you:

  1. Provide complete and accurate information. Share accurate details about your
    identity, medical history, hospitalizations, medications, dietary supplements, and current health
    concerns. Inform your care team of any changes in your health.
  2. Follow your treatment plan. Follow the treatment plans recommended by your
    physicians and other health care providers. Let your care team know immediately if you need
    clarification or do not understand your plan of care or the health instructions you are given.
  3. Participate in your care. Be an active participant in your treatment and in
    planning for continuing care with your health care provider.
  4. Communicate about pain. Be part of your pain management team. If you are
    receiving pain medications, discuss pain management options with your medical team. Use pain
    medication as prescribed and provide feedback if certain methods are not working well for you.
  5. Be respectful. Be considerate and respectful of other patients and health
    center personnel. Help control noise levels and ensure your visitors are considerate as well. Respect
    health center property.
  6. Follow health center rules. Comply with health center rules and regulations,
    including those that prohibit offensive, threatening, or abusive language or behavior, and the use of
    tobacco, alcohol, or illicit drugs on the premises. Help ensure your visitors are aware of and
    follow these rules.
  7. Share advance directives. Provide the health center with a copy of any advance
    directive or health care proxy designation you have prepared.
  8. Meet your financial obligations. Provide accurate and complete financial
    information and work with the health center to ensure financial obligations related to your care are
    met. Notify us promptly if there is a hardship so that we may assist you as needed.
