The Community Health Northwest Florida Foundation mission is to inspire charitable giving and to assist donors in providing for the care of patients through prescription assistance, diagnostic testing, and program support.
The Foundation fulfills our mission to provide a full range of health and wellness services to all who walk through our doors, and to continually strive to improve our programs and services to better serve patients in our community.
We are currently focused on health and wellness programs and services for expectant mothers, newborns, toddlers, children and seniors.
Our “WellNest” initiative covers the Baby Bucks, Toddler Tokens and Kid’s Coins programs, of which each provide services, incentives and healthcare to ensure the health and wellbeing of our patients.
Your contribution will make a tangible difference. Your gift will support our Baby Bucks program for expectant mothers by providing them with car seats, strollers, diapers, bottles and much-needed baby supplies. Donations also will help to establish Toddler Tokens, Kid’s Coins and a full-service senior wellness program.
